Clinical Rotation Planner
Clinical rotations can become difficult to manage once schedules, onboarding requirements, clinical goals, feedback, and required documentation begin to overlap.
This Clinical Rotation Planner is designed to help DNP-FNP students organize a rotation, track their own progress, and keep important information in one place.
It is a personal planning tool. It does not replace your program’s required system for documenting clinical hours, patient encounters, competencies, or evaluations.

Protect Patient Privacy
Do not record patient names, dates of birth, medical-record numbers, addresses, photographs, or other identifying information in this planner.
When documenting clinical experiences for your own learning, use only the level of detail permitted by your academic program, clinical site, and applicable privacy requirements.
If your program requires patient-encounter logging, complete that documentation in the approved system rather than reproducing identifiable clinical information here.
